WIC Forms You May Need for Your WIC Appointment
The VOC Request Form is used when you have moved, but did not obtain the VOC Card from your WIC Program. Submission of this form enables you to transfer to another WIC Program:

The Authorization Form is used when you would like to allow someone else to participate in the WIC Certification process and/or pick up WIC food instruments:

You may be asked to have these forms completed by a health care provider. These forms provide information that is needed by WIC staff at your certification:
